THE LIFE COACH and STEP FORWARD LIFE INSTITUTE AGREEMENT

I, {first_name} {last_name}, agree to use my Life Coach Certification in a manner upholding the standards and code of ethics of STEP Forward Life Institute (SFLI) and Coaching and Consulting Connections, LLC; adhering to the CARE Readiness principles upon which the program is based.

I understand that the STEP Forward Life Institute, as an affiliate of United Graduate College and Seminary International (UGCSI) is issuing me this certification, but will be held harmless in the event it is misused in any way.

It is my understanding that my certification is effective for two years and will require taking refresher or enhancement courses through the STEP Forward Life Institute to keep my certification current. Upon this renewal I may continue to use my certification.
